Changes apply to small providers with FFN subsid The Nevada Child Care Licensing unit is informing all currently licensed providers that the Consent and Release form for employee background fingerprinting has been revised. These revisions, however, apply only to those providers who care for fewer than 4 children and who receive a “friends, family and neighbors” (FFN) subsidy from Children’s Cabinet (in Northern Nevada) or Urban League (in Southern Nevada). The revised forms can be found under the “Forms – Background” orange header on the Child Care Licensing forms web page. These forms are to be used by all Nevada child care providers effective immediately, however only the FFN providers are required to fill in the information pertaining to subsidy programs. Other than this information, all providers must fill out this form completely or it will be rejected.

This form must be completed by the administrator at the HIRC’s initial license application, and by each employee of a HIRC and kept in their file for annual inspection. In signing the form, individuals associated with a licensed Nevada HIRC are stating that they follow the guidelines for safe-injection practices set forth by the Centers for Disease Control and Prevention (CDC).
